There's actually three things in there. The first is one of the numerous photos of Winry in twin braids in her underwear Haruhi took. This is placed directly on top of a picture of Haruhi in about the same outfit. She has mounted a tripod in her room clearly, because it's on her bed and trying to match the same pose.

But those are secondary. Beneath both of those is the larger and much more obvious real gift. Haruhi hasn't lived with a seamstress for years now and picked nothing up - in there is a hand-made (or well, made using equipment in All Passions, but the point is she made it out of materials on hand) swimsuit for Winry. A two piece pink (as they discussed) bikini which I am too lazy to link an image reference for. ...She knows this is probably not up to par, but she did at least include a little design motiff on it in the form of frilly things? She hangs her head a little, flustered, as she hands it over.
